Pathophysiology
Clinical meaning
Registered Practical Nurses (RPNs) operate within a defined scope of practice governed by provincial or state regulatory bodies. Understanding delegation authority is critical: RPNs must verify that any task delegated by an RN falls within their competence and scope before accepting it. Informed consent, mandatory reporting obligations, documentation standards, and confidentiality requirements form the legal backbone of safe practical nursing. Violations such as practicing outside scope, falsifying records, or breaching patient privacy on social media constitute professional misconduct and carry serious regulatory consequences including license suspension or revocation.
Exam Focus
Exam relevance
Risk factors:
- Accepting delegated tasks beyond scope of practice
- Failing to verify informed consent before procedures
- Not recognizing signs of patient incapacity
- Incomplete or late documentation without proper correction procedures
- Posting patient information on social media platforms
- Failing to report suspected abuse or communicable disease
Diagnostics:
- Assess patient capacity to provide informed consent: can the patient understand the information, appreciate how it applies to them, reason about options, and communicate a decision?
- Verify the presence of a valid informed consent form signed by the patient or substitute decision-maker before assisting with any invasive procedure
- Screen for indicators of potential abuse: unexplained injuries, inconsistent explanations, withdrawal, fear of caregiver, neglect of hygiene or nutrition
- Review documentation for completeness and legal defensibility: are entries factual, objective, timely, and free of subjective opinions or blame?
- Assess for situations requiring mandatory reporting: suspected child abuse, elder abuse, neglect, communicable disease, or fitness-to-drive concerns
- Evaluate whether a delegated task falls within RPN scope by consulting the regulatory body's practice standards and authorized acts
