Clinical meaning
The comprehensive History and Physical (H&P) is the foundational clinical skill for NP-level independent practice, integrating systematic data collection with advanced clinical reasoning to formulate differential diagnoses, guide diagnostic workup, and direct evidence-based management. The NP performs a structured history beginning with chief complaint, history of present illness (characterized using OLDCARTS: Onset, Location, Duration, Character, Aggravating/Alleviating factors, Radiation, Timing, Severity), comprehensive past medical/surgical history, medications with adherence assessment, allergies with reaction type documentation, family history for genetic risk stratification, social history (tobacco in pack-years, alcohol in drinks/week, substance use, occupation, sexual history, exercise, diet, living situation, advance directives), and complete review of systems. The physical examination progresses head-to-toe using inspection, palpation, percussion, and auscultation (IPPA; abdomen uses inspection, auscultation, percussion, palpation sequence) with integration of advanced techniques: JVP estimation at 45 degrees, hepatojugular reflux, point-of-maximal-impulse characterization, provocative maneuvers for musculoskeletal pathology (McMurray, Lachman, straight leg raise), neurological screening (cranial nerves, deep tendon reflexes, cerebellar testing, sensory modalities), and mental status evaluation. The NP synthesizes subjective and objective findings through clinical reasoning — generating a problem list, formulating differential diagnoses ranked by probability, identifying red flags, and developing assessment and plan for each active problem.
