Clinical meaning
Takotsubo (stress) cardiomyopathy results from a massive catecholamine surge that causes direct myocardial toxicity and microvascular spasm. The apical segments develop transient wall motion abnormalities (apical ballooning) while basal segments hypercontract, giving the ventricle a characteristic 'takotsubo' (Japanese octopus trap) shape.
Diagnosis & workup
Diagnostics & workup: - Order 12-lead ECG (may show ST elevation mimicking STEMI) - Order serial troponin (mildly elevated, disproportionate to wall motion) - Order emergent coronary angiography to exclude MI - Order echocardiogram showing apical ballooning with basal hyperkinesis - Order BNP (elevated) - Order ventriculography during catheterization
Risk factors: - Postmenopausal women (90% of cases) - Acute emotional stress (grief, surprise, argument) - Acute physical stress (surgery, critical illness) - Pheochromocytoma - Neurological injury (subarachnoid hemorrhage)
