Pharmacologic Class: Mast cell stabilizers (cromones) Agents: - Cromolyn sodium (Intal — discontinued in US as MDI; still available as nebulizer solution and nasal spray) - Nedocromil (Alocril — ophthalmic use; Tilade — respiratory use; limited availability) Mechanism: - Inhibits mast cell degranulation → prevents release of histamine, leukotrienes, prostaglandins - Mechanism not fully understood; may block chloride channels → prevents calcium influx → prevents exocytosis of mediator granules - Also: inhibits sensory nerve activation → reduces neurogenic inflammation - Does NOT cause bronchodilation — purely preventive/prophylactic - DOES NOT treat acute bronchospasm Current Role: - Largely supplanted by ICS (superior efficacy); cromolyn now a niche agent - May still be used: mild persistent asthma in young children as ICS alternative; exercise-induced bronchospasm prophylaxis; allergic rhinitis (nasal form) - Must be taken regularly — onset of action 2–4 weeks for full effect - Safe in pregnancy (Category B) On the exam, writers often pair stable-sounding options with unstable data—notice the mismatch before you commit.
