Why therapeutic communication is tested across all NCLEX-RN domains: Communication is not a separate topic โ it appears in mental health, medical-surgical, pediatric, maternal, and leadership questions. The RN must select the MOST therapeutic response from options that may all sound polite and helpful. NCLEX tests a specific framework: responses that facilitate patient expression and maintain the therapeutic relationship vs. responses that shut down communication, impose judgment, or give false reassurance. Therapeutic communication goals: - Build trust within the nurse-client relationship - Facilitate expression of thoughts, feelings, and concerns - Gather accurate assessment data - Provide emotional support without creating dependency - Help the patient process difficult emotions and decisions The nurse-client relationship phases (Peplau): 1. Orientation: Patient identifies needs; nurse establishes trust and roles 2. Working (identification): Patient actively works on problems with nurse's guidance; most of the therapeutic work occurs here 3.
