High-Alert Medication Double Checks

One incorrect concentration, calculation, or pump setting can turn a routine medication dose into rapid harm. High-alert medications are identified because the consequences of an error can be severe, not because an error is guaranteed to occur. For an RPN, the double check is an active safety barrier, not a signature added after the work is done. Before preparation and/or administration, as required by the applicable policy, an independent colleague who was not involved in preparing the medication verifies the order and the medication details. The verification is documented in the medication record. This second check does not replace your own assessment or medication rights. You remain responsible for confirming the client, order, medication, concentration, dose, route, timing, required monitoring, and your authority to administer it. If a required checker is unavailable or any detail does not match, pause and follow the escalation process rather than creating an informal workaround.
