Why client safety is a priority on REx-PN: Falls are the most common adverse event in Canadian healthcare facilities. Restraints carry significant harm risk and are strictly regulated in Canada. The PN must perform evidence-based fall risk assessment, implement preventive interventions, apply the "least restraint" principle, and maintain patient dignity throughout. These are core REx-PN competencies. Falls statistics in Canada: - Falls are the leading cause of injury-related deaths in older adults in Canada - Hospital falls affect 1 in 4 patients over 65 in acute care - Fall-related injuries cause pain, functional decline, prolonged hospitalization, and loss of confidence - Hospitals and LTC facilities are legally and professionally accountable for fall prevention Least restraint principle (Canadian standard): The default position in Canadian nursing is that physical restraints are NOT used. If a restraint is needed, the least restrictive option is used for the shortest time possible.
