- Order CT head for suspected traumatic brain injury (repeated concussions from physical assault)
- Order cervical spine imaging if strangulation with neck pain is reported
- Order comprehensive labs: CBC, CMP, coagulation studies, pregnancy test, STI panel
- Screen for PTSD using PCL-5 (score ≥33 suggests PTSD), depression using PHQ-9, and anxiety using GAD-7
- Assess for substance use disorders using validated screening tools (AUDIT, DAST)

- Substance use disorder in either partner (particularly stimulants and alcohol)
- Partner's access to weapons (firearms increase homicide risk 5-12x)
- Recent separation or restraining order (highest lethality period)
- Pregnancy (IPV affects 3-9% of pregnancies; homicide is a leading cause of maternal mortality)
- History of strangulation (increases homicide risk 7x)
- Partner with prior criminal history or mental health disorders
