Neutropenia changes the meaning of ordinary assessment findings. When circulating neutrophils are depleted, a patient can have a dangerous bacterial or fungal infection without pus, marked redness, or a focal infiltrate. ANC and temperature therefore drive urgency: an ANC below 500 cells/µL is severe, and fever in this setting requires rapid evaluation and empiric broad-spectrum therapy. Neutropenic patients receive standard precautions plus contact precautions; those undergoing hematopoietic stem-cell transplantation require a protective environment with HEPA filtration. These measures reduce exposure but never replace cultures, treatment, or reassessment.
