Why the Neck Veins Matter

The neck veins can reveal pressure building behind the right side of the heart before the rest of the examination makes the pattern obvious. A raised jugular venous pressure (JVP) supports systemic venous congestion; in a patient with breathlessness, it increases the likelihood of heart failure, but it does not prove it. The internal jugular vein is connected to the superior vena cava and right atrium through an uninterrupted, valveless fluid column. The vertical height of its venous column therefore estimates right-atrial filling pressure, often called central venous pressure. That estimate reflects right-sided pressure and venous congestion—not left-sided pressure, total body water, or the patient’s ability to respond to an IV fluid bolus by increasing cardiac output. The experienced nurse does not read JVP in isolation. The meaning changes with blood pressure, lung findings, peripheral oedema, urine output, renal function, oxygen requirement, and the reason the patient is unwell. A prominent JVP with hypotension may signal obstructive shock rather than simple fluid excess.
