Anticoagulation and Thrombolytic Safety

Balancing clot prevention with bleeding risk is a constant bedside calculus. An RN must recognise when a medication is doing its job, when it has over‑stepped, and how quickly the therapy can be halted or reversed. The stakes are highest with direct oral anticoagulants (DOACs) and the thrombolytic alteplase, because dosing errors or missed contraindications can precipitate fatal intracranial hemorrhage or uncontrolled thrombosis.
