Laboratory test interpretation in NP practice requires understanding of pre-test probability, test characteristics (sensitivity, specificity, positive and negative predictive values), and Bayesian reasoning to move from clinical suspicion to diagnostic confirmation. A confirmatory test has high specificity -- it rarely produces false positives, so a positive result reliably rules IN the diagnosis (SpIN: Specific test, Positive result, rules IN). A screening test has high sensitivity -- it rarely produces false negatives, so a negative result reliably rules OUT the diagnosis (SnNOUT: Sensitive test, Negative result, rules OUT). Key confirmatory laboratory patterns include: Troponin I/T for acute myocardial infarction (highly sensitive and specific for myocardial necrosis; serial measurements at 0, 3, and 6 hours with high-sensitivity troponin assays; rising and/or falling pattern confirms acute MI).
