Community health nursing begins with a population, not a diagnosis. The unit of care may be a neighborhood, school, workplace, or other defined group. An individual assessment still matters, but the clinical question extends beyond what one patient needs: what pattern of risk, exposure, access, or social circumstance is affecting this population, and what intervention could change it? US public health practice organizes this work around three functions: assessment, policy development, and assurance. Assessment identifies health problems and their distribution; policy development converts evidence into priorities and protective action; assurance checks whether people can actually obtain effective, safe services. The 10 Essential Public Health Services operationalize these functions. Prevention is classified by timing. Primary prevention acts before disease occurs, such as immunization or tobacco-control policy. Secondary prevention identifies disease during an asymptomatic stage through screening or targeted testing. Tertiary prevention begins after diagnosis and limits disability or progression, such as rehabilitation or diabetic foot surveillance.
