Bleeding risk is therefore not limited to a visible hemorrhage; it includes occult blood loss, medication accumulation, drug interactions, and trauma that initially looks minor. Apixaban provides a useful model for this topic. The nurse must continually balance two hazards: inadequate anticoagulation can permit stroke or recurrent thrombosis, while excessive anticoagulant effect can produce gastrointestinal, urinary, retroperitoneal, or intracranial bleeding. The first clinical question is not simply whether the patient has bruising. It is whether bleeding is active or clinically significant and whether perfusion, oxygen delivery, or neurologic function is threatened. A stable, small bruise at an injection site is assessed and documented. Melena with dizziness, a falling blood pressure, new confusion, or a head injury in a patient taking apixaban changes the priority to urgent evaluation and escalation. The finding matters because of what it may represent, not because all bleeding-related findings carry the same urgency.
