The testes are paired ovoid glands, each roughly 4 x 3 x 2 cm and weighing 15-25 g, suspended within the scrotal sac outside the body cavity. This external position is not anatomical accident: the scrotum keeps testicular tissue 2-4 degrees Celsius below core body temperature, a thermal requirement that is absolutely essential for normal spermatogenesis. The testes perform two interdependent jobs. First, they carry out spermatogenesis, the production of sperm within the seminiferous tubules. Second, they perform steroidogenesis, the synthesis of testosterone by the interstitial Leydig cells. These two functions are coordinated by the hypothalamic-pituitary-gonadal axis, in which gonadotropin-releasing hormone drives pituitary release of luteinizing hormone (LH) and follicle-stimulating hormone (FSH). For the NCLEX-RN learner, the testes matter clinically far beyond fertility counseling.
