REx-PN
A functional ovarian cyst forms when a normal ovarian follicle does not resolve as expected during the ovulatory cycle. The follicle may continue to collect fluid, producing a fluid-filled structure within or on the ovary. These cysts are most common during the reproductive years and are usually self-limiting. A simple cyst is typically anechoic, meaning it appears black on ultrasound because it contains fluid, and has a thin wall. That appearance supports a functional process. The word benign, however, does not mean that every cyst can be dismissed. Size, symptoms, ultrasound morphology, and menopausal status determine whether observation remains appropriate. A small, simple, symptom-free cyst in a premenopausal client often needs time and planned reassessment rather than immediate intervention. Pain, pressure, or bloating changes that plan because the cyst is now clinically significant, even if it began with a simple appearance. Septations, solid areas, or papillary projections also move the finding away from a reassuring simple cyst and toward specialist assessment.
