The safe choice matches a likely bacterial infection with a drug, dose, route, and duration that fit the patient's allergy history, renal function, and clinical stability. The nurse then looks for evidence that the infection is improving and for evidence that the drug is causing harm. On the REx-PN, a routine oral dose may be a distraction. A new respiratory change, a documented penicillin hypersensitivity, a creatinine clearance below the dosing threshold, or an order outside the RPN's authority changes the priority.
