What OAE and AABR Actually Measure

Automated transient-evoked otoacoustic emissions (ATEOAE) are the first-line newborn hearing screen. A small probe presents sound in the ear canal and records the echo generated by functioning cochlear outer hair cells. The test therefore samples cochlear function, not the entire auditory pathway. Automated auditory brainstem response (AABR) uses sounds delivered through earphones and electrodes on the infant’s skin to detect electrical activity as the signal travels through the auditory nerve and brainstem. It evaluates a broader part of the hearing pathway than OAE, but it remains a screening test rather than a complete diagnostic assessment. A non-pass does not prove permanent hearing loss. Vernix in the ear canal, middle-ear fluid, movement, or environmental noise can interfere with an OAE recording. That possibility explains why further testing is required; it does not justify dismissing the result. The experienced nurse treats the screen as a decision point, not as a diagnosis.
