Why Rabies Exposure Is Time-Sensitive
Rabies is caused by a lyssavirus. After inoculation, the virus can replicate in local muscle tissue, enter peripheral nerves, and travel toward the central nervous system.
Rabies is caused by a lyssavirus. After inoculation, the virus can replicate in local muscle tissue, enter peripheral nerves, and travel toward the central nervous system. Once clinical rabies develops, encephalomyelitis is almost invariably fatal. This sequence explains why post-exposure prophylaxis (PEP) is given before symptoms appear. Thorough cleansing reduces the amount of virus at the entry site. Rabies immune globulin (RIG) provides immediate passive antibody in and around the wound, while vaccine stimulates the patient’s own antibody response over the following days. PEP prevents disease; it does not reliably treat symptomatic rabies. The nurse’s first instinct after a possible exposure should be practical rather than diagnostic: identify what contacted the patient, cleanse the site, and obtain an urgent public health risk assessment. A tiny or invisible wound does not make a potentially significant exposure safe.
