What this calculator does

The intake and output calculator totals fluid intake and output, computes net balance, and, when weight and time are entered, urine output in mL/kg/hr. It is a study tool — interpret balance with assessment findings, orders, and institutional policy.

Formulas & why trends matter

Net balance = total intake − total output. Urine mL/kg/hr = urine ÷ weight (kg) ÷ hours.

A single balance value means little on its own. Connect the trend to dehydration, fluid overload, acute kidney injury, heart failure, sepsis, and post-op monitoring, and escalate per policy when the findings and the numbers disagree.

Step-by-step example

Intake 1,200 mL, output 900 mL over a shift.

  1. Net balance = 1,200 − 900 = +300 mL (positive balance).
  2. If the patient weighs 60 kg over 8 hr with 480 mL urine: 480 ÷ 60 ÷ 8 = 1.0 mL/kg/hr.
  3. Interpret alongside weight trend, edema, respiratory status, and diagnosis.

Common mistakes

  • Missing sources such as flushes, medication volumes, or drain output.
  • Treating one balance value as a diagnosis instead of a trend to interpret.

Frequently asked questions

What counts as intake and output?+

Intake includes oral fluids, IV fluids, tube feeds, flushes, blood products, TPN, and medication volumes. Output includes urine, emesis, drains, stool or ostomy output, wound drainage, and NG output.

How do you calculate net fluid balance?+

Subtract total output from total intake: net balance = total intake − total output. A positive number means intake exceeded output.

How do you calculate urine output in mL/kg/hr?+

Divide urine output by the weight in kilograms and then by hours: urine mL/kg/hr = urine ÷ weight (kg) ÷ hours.

Why do fluid balance trends matter more than one value?+
